Parochial Reports

All Episcopal congregations are required to complete an annual statistical report called the Parochial Report. The report is established as a tool for the collection of data that is intended to assist the Church in planning for mission.

The report is divided into five pages: (1) Certification and Approval, (2) Membership, Attendance, and Services, (3) Stewardship and Financial Information, (4) Priests and Deacons serving the congregation, and (5) Outreach Ministries and Volunteer Activity of this Congregation. 

In addition, as many other dioceses do, the Diocese of Kentucky includes another report for congregations to complete called the Diocese's Congregational Assets, Liabilities, and Insurance report

Parochial Reports are sent to the church offices and posted on the web in early January and are due by March 1st. The reports must be approved by the church's Vestry or Bishop's Committee. To request an extension for filing, please contact the Secretary of the Diocese as soon as possible.

2025 Parochial Report Information and Instructions

The Episcopal Church Parochial Report website – http://reports.dfms.org – is now open for filing reports, which are due March 1, 2025. Line-by-line instructions, including a helpful video, are available on the General Convention website here.  

The Diocese of Kentucky has an additional form: Report of Congregation’s Assets, Liabilities and Insurance. It is called the Supplemental Page this year and can be found here. This form is not on the Episcopal Church website, so please scan and email it to me at kayswilkinson@msn.com or send it to me through regular U.S. mail at
